Accutane (Isotretinoin) 20mg capsules are the definitive pharmaceutical treatment for severe cystic acne, including steroid-induced acne that does not respond to topical retinoids or antibiotics. Isotretinoin is the only acne medication that addresses all four causes simultaneously: excess sebum production (reduced by up to 90%), follicular hyperkeratinisation, bacterial colonisation, and inflammation. For bodybuilders and steroid users, low-dose Accutane (10-20mg/day) is typically sufficient to control androgen-driven breakouts without the severe side effects seen at full dermatological doses.
Why Steroid Users Need Accutane
Androgenic steroids (Testosterone, Dianabol, Trenbolone, Anadrol) dramatically increase sebaceous gland activity. The excess sebum creates an ideal environment for acne-causing bacteria, leading to breakouts ranging from facial acne to severe cystic bacne (back acne) across the shoulders, chest, and back. When standard treatments (benzoyl peroxide, salicylic acid, topical antibiotics, Tretinoin cream) fail to control the breakouts, oral Isotretinoin is the escalation.
Low-Dose Protocol for Steroid Acne
- Dose: 10-20mg/day (significantly lower than the 0.5-1mg/kg standard dermatological dose)
- Duration: 4-6 months, or concurrently with steroid cycles (injectable-only cycles)
- Take with fat-containing food: absorption doubles when taken with dietary fat
- Do NOT combine with C17-aa oral steroids (Dianabol, Winstrol, Stanavar). Both stress the liver. Wait until the oral portion of your cycle is complete before starting Accutane
- Liver monitoring: ALT/AST and lipid panel every 4-6 weeks during treatment
Side Effects
- Dry skin and lips: universal. Moisturiser and lip balm are essential throughout
- Dry eyes: artificial tears recommended for contact lens wearers
- Joint and muscle pain: 15-20% of users. Can be exacerbated by heavy training
- Elevated liver enzymes: monitoring required, especially if combining with injectable steroids
- Mood changes: small percentage of users. Discontinue if significant
- Severely teratogenic: causes birth defects. Female users must use reliable contraception
