Sustanon 250mg is a blend of four testosterone esters designed to provide both fast-acting and sustained testosterone release from a single injection. Originally developed by Organon for testosterone replacement therapy, Sustanon combines Testosterone Propionate (30mg), Phenylpropionate (60mg), Isocaproate (60mg), and Decanoate (100mg) totalling 250mg of testosterone per millilitre. This multi-ester design delivers rapid onset within 24-48 hours from the short esters while the longer esters extend the active window to 2-3 weeks.
How the Four Esters Work Together
Each ester releases at a different rate, creating a staggered testosterone release profile:
- Testosterone Propionate (30mg): peaks in 24-48 hours. Provides immediate testosterone elevation
- Testosterone Phenylpropionate (60mg): peaks in 1-3 days. Bridges the gap between Propionate and the longer esters
- Testosterone Isocaproate (60mg): peaks in 4-7 days. Maintains mid-cycle blood levels
- Testosterone Decanoate (100mg): peaks in 7-14 days. Provides the sustained base
Despite this multi-ester design, twice-weekly injections (e.g., Monday and Thursday) still produce more stable blood levels than a single weekly shot. The Propionate component creates a sharp initial peak that drops off quickly, so splitting the dose across two injections smooths out the highs and lows.
Suggested Protocol
- Bulking dose: 250-500mg per week (1-2ml) for 10-14 weeks
- Injection frequency: 2x per week for optimal stability despite the multi-ester design
- On-cycle AI: Aromasin or Arimidex as needed. Sustanon aromatises identically to any other testosterone form
- PCT start: 18-21 days after last injection (the Decanoate ester has a 15-day half-life and takes longer to clear than Enanthate)
- PCT: Nolvadex 40/40/20/20mg for 4 weeks
Sustanon vs Testosterone Enanthate
- Sustanon: faster initial onset (Propionate component). Slightly more complex pharmacokinetics. Longer PCT wait (18-21 days vs 14 days). More injection site soreness for some users. Historically more expensive
- Test Enanthate: simpler single-ester pharmacokinetics. More predictable blood levels with twice-weekly injections. Faster PCT initiation (14 days). Typically smoother injections. More cost-effective at equivalent doses
- Bottom line: both deliver identical testosterone to the body. Enanthate is generally preferred for simplicity. Sustanon is preferred by users who like the multi-ester concept or who are switching from TRT (where Sustanon is commonly prescribed in the UK)
