Accutane (Isotretinoin) 20mg is the most effective treatment for severe and cystic acne, including steroid-induced acne that does not respond to topical treatments or antibiotics. Isotretinoin is a retinoid derived from Vitamin A that works by dramatically reducing sebaceous gland size and sebum production by up to 90%, according to dermatological data published in the Journal of the American Academy of Dermatology. It is the only acne medication that addresses all four pathogenic factors: excess sebum, follicular keratinisation, bacterial colonisation (C. acnes), and inflammation.
Why Steroid Users Need Accutane
Anabolic steroids, particularly testosterone and other highly androgenic compounds, stimulate the sebaceous (oil-producing) glands in the skin. This increased sebum production creates an ideal environment for acne-causing bacteria, leading to breakouts that range from mild facial acne to severe cystic acne across the back, shoulders, and chest (“bacne”). For many users, the acne does not respond to standard treatments like benzoyl peroxide, salicylic acid, or topical antibiotics.
Accutane is the definitive solution for steroid-induced acne that has failed to respond to first-line treatments. A low-dose protocol (10-20mg/day) is typically sufficient for steroid-related acne, as opposed to the higher 0.5-1mg/kg doses used in standard dermatological practice for severe acne in non-steroid users.
Dosage for Steroid-Related Acne
- Low-dose protocol: 10-20mg/day (most effective for steroid-induced acne without the severe side effects of full-dose treatment)
- Standard dermatological dose: 0.5-1mg per kg of bodyweight per day (used for severe non-steroid acne; more side effects)
- Duration: 4-6 months for a full course. Low-dose protocols may run concurrently with steroid cycles
- Take with a fat-containing meal: Isotretinoin is fat-soluble. Absorption increases by approximately 2x when taken with dietary fat
- Onset: initial worsening (“purge”) in weeks 1-4 is common. Significant improvement typically begins by week 6-8
Side Effects and Monitoring
Isotretinoin is a potent medication with well-documented side effects that require monitoring:
- Dry skin and lips: universal. Moisturiser and lip balm are essential throughout treatment
- Dry eyes: common. Artificial tears recommended, especially for contact lens wearers
- Joint and muscle pain: reported by 15-20% of users. Can be exacerbated by heavy training. Reduce volume if significant
- Elevated liver enzymes: Isotretinoin stresses the liver. This is particularly important for users also taking C17-aa oral steroids (Dianabol, Winstrol, Anavar). Avoid running Accutane concurrently with hepatotoxic oral steroids. Liver panel monitoring every 4-6 weeks is recommended
- Elevated triglycerides: lipid monitoring required. Omega-3 supplementation recommended
- Mood changes: depression and mood alterations have been reported in a small percentage of users. Discontinue and seek medical advice if significant mood changes occur
- Pregnancy risk: Isotretinoin is severely teratogenic (causes birth defects). Female users must use reliable contraception throughout treatment and for 1 month after stopping
Important: Do Not Combine With Oral Steroids
Both Isotretinoin and C17-alpha alkylated oral steroids (Dianabol, Winstrol, Stanavar) stress the liver. Running them simultaneously creates compounded hepatotoxicity. If you are on an oral steroid cycle, wait until the oral portion is complete and liver values have normalised before beginning Accutane. Injectable steroids do not pose this conflict.
